JOB TITLE: Maui Recovery Program Case Manager CLASSIFICATION: Non-Exempt Reports to: Maui Recovery Project Coordinator This is a new position in the Emergency Disaster Services Department.
Position not to exceed (3) years with the option to extend based on workload and funding.
SUMMARY: Under the general supervision of the Maui Recovery Project Coordinator (MRPC), the Maui Recovery Project Case Manager (MRPCM) provides case management for identified individual/family survivors (clients) in relation to The Salvation Army Maui Recovery Project assistance/services.
These services include rapid response contact with clients and periodic repeated contacts, including local travel, to meet clients’ material needs, collaboratively set client recovery goals and plans, inform clients of emerging resources and resolve system impediments, provide referrals to other disaster and non-disaster services.
WORKED PERFORMED: Provide disaster recovery case management services to persons temporarily residing in community settings including motels, shared homes, and other housing situations.
These case management services and assistances would be made available to survivors, based on resources identified with community partners, through the disaster case management program, through the long-term recovery group, and/or through the unmet needs table.
Determine eligibility and assess survivors’ disaster related needs utilizing the Salvation Army established criteria or disaster case management program criteria (this could include special populations assistance as identified through the long term recovery group efforts as well), complete disaster intake form(s) and records, provide disaster related financial assistance, and record disaster related case management services according to prescribed unit guidelines, including regular updates to case files, and any coordinated network management information system, and other documentation mechanisms.
Implement long-term case management planning services, which will include individualized disaster recovery plans, with disaster survivors leading to the elimination of disaster-related needs; setting mutually agreeable client recovery goals to restore client to previous level of functioning; and regularly monitor client progress through regular communication with the client to identify action steps completed, barriers encountered, and outcomes achieved.
Input client and client related information (including case notes) into The Salvation Army WellSky social service database.
Information will be entered in a timely manner and with accuracy.
Data may also be entered into any related coordinated network information system.
Provide disaster and non-disaster information and referral services, assist clients to understand the resources available to them and refer clients to resources, facilitate clients to obtain the needed resources, and track their participation in these resources.
Assist as a liaison between clients, The Salvation Army, and other agencies when necessary.
Record agency spending on clients and provide timely and correct statistical data and reports, in collaboration with the Maui Recovery Program Administrative Assistant and the Maui Recovery Project Coordinator.
Maintain confidentiality, comply/enforce proper client boundaries, adhere to TSA Social Service Code of Ethics, and comply with Protecting the Mission Guidelines.

Minimum Qualifications: Bachelor’s degree with one-year related work experience.
Combination of training and experience will be considered.
(MSW with work experience with undocumented, homeless, kupuna, and/or other specialized populations is preferred.
) Must have strong communication skills and an ability to work with diverse cultures.
Strong leadership style and great organizational skills required.
Commitment to carry out The Salvation Army mission is essential.
Knowledge of The Salvation Army and Government, State and community resources is helpful.
Must have reliable transportation, a valid Driver’s License and pass The Salvation Army Motor Vehicle Report screening.
Physical and Work Environment Requirements: Must be capable of performing the above essential duties in an office environment to include using a computer, answering phones, etc.
Occasionally lifts 24 pounds.
Most work will be indoors in a temperature control environment.
However, occasional outdoor travel is required in the summer and winter.
Background Checks: Position requires background checks to be accomplished.
Findings may disqualify an individual for this position.
OTHER FUNCTIONS: Performs other duties as assigned.
SKILLS/TECHNICAL KNOWLEDGE: Valid driver’s license Prioritize multiple work tasks to meet tight deadlines.
Excellent organizational skills Solid understanding and regular use of Microsoft Office Able to work on short-term and long-term projects with competing deadlines.
