The Position:
We're looking for a passionate Director of Rehabilitation to provide leadership and direction to the OPH Rehabilitation division.
The Director of Rehab (DOR) is responsible for developing and implementing strategic and innovative initiatives to sustain and grow the program while following all Federal, State, and other pertinent rules and regulations.
The DOR works collaboratively with the Nursing Clinical Managers, Patient Care Coordinators, and QAPI Manager to ensure the patient receives coordinated, patient-centered care.
The DOR ensures therapy services and all care to the patients are provided in accordance with patient and family goals, professional, legal, regulatory, accrediting, and administrative requirements and follows the performance improvement plan for the division.
Oversight of the services would include, but not limited to, providing a thorough orientation training program, assigning clinicians to patients, coordinating care, supporting the clinicians through joint visits or telephony, coordinating between physicians, vendors, patients/families and OPH.
Coordinating referrals, coordinating assignments, assisting with continually assessing clinicians’ plans for the patients to ensure the quality and safety of the patient.
Managing the complex, interdisciplinary care of patients by assisting the clinicians in resolving issues they encounter in the home.
Provide evidence-based therapy education and training
Requirements:
Bachelor's degree in Physical therapy, Occupational therapy or Speech Language Pathology; OR
Master's degree in Physical, Occupational or Speech Language Pathology; OR
Doctorate Degree in Physical Therapy or other healthcare related field.
Master of Business Administration, Master of Healthcare Administration, or other master level of health-related degree preferred.
Current medical/health screening including 2 step TB clearance
Recent two years of home health experience.
Two years home health management experience.
Two years supervisory experience (clinical oversight, recruitment, competency evaluation, etc.
)
Medical community relationships through previous career experiences.
