JOB SUMMARY/RESPONSIBILITIES: Under the general supervision of the Clinical Manager or the designee, assesses, plans, implements, coordinates, monitors and evaluates options and services required to meet the needs of patients to promote quality cost-effective care by helping the Medicare Certified Services (MC) staff.
Ensures that specific outcomes are achieved and that variances from established and accepted parameters are evaluated and addressed, as necessary.
The Team Leader is knowledgeable in community health nursing and/or rehabilitation concepts and can work independently and communicate effectively with clients, staff, and providers.
Providing lead direction to professional, technical and support staff, and providing case management services for CareResource Hawaii (CRH) patient.

Performs in accordance with policies and procedures of CRH and The Queen’s Health System.
II.
PRIMARY DUTIES AND RESPONSIBILITIES: OPERATIONAL RESPONSIBILITIES: Provides leadership to the MC staff, collaborates, and communicates with the Clinical Manager, acts as a positive role model, and provides effective supervision to ensure good team working relationship.
Motivates, supports, supervises, mentors, and collaborates with the clinical staff.
Maintains oversight for continual delivery of services, case management, and provides services as needed.
Coordinates and participates in case conference.
Plans and implements strategies to improve and ensure quality and cost-effective care of patients.
The RN (Registered Nurse) Team Leader assists with medication oversight for the therapy only cases as per Conditions of Participation (COP).
Ensures that orders, including the initial plan of treatment, re-certification, and ongoing orders, are obtained as needed and OASIS (Outcome and Assessment System Information Set) documents are completed as required.
Participates in the management of the Medicare Certified Services Division telehealth program.
Participates in the CRH’s performance improvement program, including audits/reviews, responding to Additional Document Requests (ADRs), working with clinicians on all requirements for Medicare and other contracted vendors as well as OASIS documentation requirements, and all other strategic performance improvement goals.
Prospective review of OASIS assessments to includes the Start of Care, Resumption of Care, Recertification, and Discharge OAISIS, to ensure appropriateness, and compliance with state and federal regulations.
Participates in the patient admission process and ensures appropriateness of patient admission by collaborating with the Clinical Managers and team members to determine if admission meets home health criteria.
Attends staff meetings, in-services, interdisciplinary case conferences and develops a cohesive team concentrating on providing quality care for all patients.
Assists with training and precepting of new clinicians as appropriate.
Performs admissions and visits as assigned.
OVERSEES/RESPONSIBLE FOR ADMINISTRATIVE FUNCTIONS: Implements plans to achieve goals and objectives for area(s) assigned.
Assists in implementing continuous performance improvement plans for department to achieve operational efficiencies and quality standards.
Recommends, implements, monitors, keeps current knowledge of department(s) and systems policies and procedures.
MANAGEMENT OF PROGRAMS/PROJECTS/INITIATIVES: Leads and/or participates in projects, programs, and initiatives as assigned.
STAFF OVERSIGHT: Oversees staff by observation, training, providing supervisory visits, and reports all information and documentation to the Clinical Managers.
Performs other related duties as assigned.

Operates various office equipment such as computers, laptops, cell phones, copiers, facsimile, telephone, etc.
Uses medical equipment like walking aides, bath equipment, bedside commode, adaptive devices, BP cuffs, thermometers, pulse oximeters, Hoyer lifts, electric beds, wheelchairs, etc.

TYPICAL WORKING CONDITIONS: Subjected to adverse environmental conditions like continuous exposure to blood borne pathogens.

MINIMUM QUALIFICATIONS: A.
EDUCATION/CERTIFICATION AND LICENSURE: Associate or bachelor's degree in nursing; or bachelor's degree in occupational therapy; or master's or Doctorate in Physical Therapy; or master's degree in Speech Language Pathology Valid State of Hawaii license to practice as a Registered Nurse, Physical Therapist, Occupational Therapist, or a Speech Therapist.
Valid U.
S driver’s license, vehicle and vehicle insurance as required by State Law.
Current Basic Life Support CPR (cardiopulmonary resuscitation) certification through the American Heart Association B.
EXPERIENCE: In addition to the educational requirement, two (2) years’ experience as a home health clinician visiting and caring for patients.
