About the Role:
DWGAS is seeking an Advanced Practice Registered Nurse Practitioner professional who has provided services in community-based settings to join our team! This individual will report to the Medical Director and work closely with the Quality and UM management team, as part of the medical team that provides healthcare services to the Hawaii population.
The ideal candidate will conduct health and wellness assessments and establish and coordinate appropriate care plans with patients.
This position will provide medical assessments, telehealth AWV, exams, health education/literacy, referrals, and health screenings in a wide variety of settings.
The Nurse Practitioner shall be responsible for managing health problems and healthcare for patients in accordance with State and Federal rules and regulations and the nursing standards of care including, but not limited to: assessment of health status, diagnosis, complete individual care plans, delivering clinical care programs, implementation of the treatment plan, evaluation of patient status, dissemination of patient health education and other benefit enhancement programs.
Essential Duties and Responsibilities:
● The APRN/RN or Nurse Practitioner functions as an expert practitioner in collaboration with other health care professionals externally and internally to enhance the quality of patient care.
● Provides health care that includes services to individuals and families that emphasize health promotion, disease prevention, and diagnosis and management of acute and chronic diseases
● Direct care services may include, but are not limited to, obtaining a patient history, providing a complete or focused physical examination, ordering, conducting, and interpreting diagnostic and laboratory tests, prescribing medications, patient and family education, treatment plans, and dealing with social, economic, and ethical issues within the scope of a healthcare provider on behalf of the medical group.
● Collaborate with Providers, Quality and UM as appropriate, serves as a health care researcher, interdisciplinary consultant, patient advocate, and clinical care extension support to drive value-based care outcomes.
● Direct patient care via telehealth consultations, home visits, or in the provider office setting that may include, but not limited to, obtaining medical history, providing a complete or focused physical exam, ordering, conducting, and interpreting/analyzing diagnostic, med recon and laboratory tests
● Examine patients and perform patient workup and assessment of patient's care such as prescribing medication treatment(s), determining the need for and ordering laboratory tests, X-rays, and other procedures necessary to support the Medial Group Care programs.
● Assisting/coordinating timely post-facility follow-up
● Operates various office and medical equipment, including personal computers
● Perform wound care procedures as deemed necessary
● Support and collaborate with QI and UM to meet Quality and Care Programs
● Any assignments or projects assigned by the CEO or Supervisor.
Core Requirements:
● APRN/RN License to practice in the State of Hawaii
● BLS Certification (Required)
● Master's (Preferred)
● Minimum of 3 years of experience in relevant health or related field
● Minimum one year (12 months) of experience as an APRN or clinical RN
● Full vaccination against COVID-19
* Bilingual skills: Tagalog or Ilocano speaking preferred.
● Some travel may be required
