*Essential Duties and Responsibilities*:
* The Admissions Coordinator/Charge Nurse oversees patient admissions and works with the nursing team in providing care.
* Provides clinical liaison services between short-term sub-acute inpatient facility and referring hospitals.
Works closely with hospital case managers/discharge planners to identify appropriate patients for the sub-acute facility.
* Makes in-person visits to hospitals to meet with case managers and potential patients.
* Provide direct patient care in accordance with established nursing care plans.
* Assist in training new staff.
* Ensures compliance with health and safety regulations
* Administer medications and treatments as prescribed by the physicians.
* Collaborate with the interdisciplinary healthcare team to develop and implement individualized care plans.
* Maintain accurate documentation.
*Minimum Qualifications:*
* Must hold and maintain a current license to practice as a Licensed Practical Nurse or Registered Nurse in the State of Hawaii.
* Recent clinical experience, education, or specialty skills specific to geriatrics.
* Demonstrate understanding that state and Federal rules and regulations govern the practices within the facility.
* Clinical assessment and chart review experience is required.
* Recent clinical experience, education, or specialty skills specific to geriatrics.
* Must be self-motivated.
* Strong communication and interpersonal skills to effectively interact with patients, families, teammates, and healthcare professionals.
* Ability to prioritize tasks and manage time effectively.
* Basic computer knowledge and ability with an aptitude to learn company software.
* Must be capable of maintaining regular attendance.
* Must meet all local health regulations and pass post-employment physical exam, if required.
This requirement also includes drug screening, criminal background investigation and reference inquiry.
