As the Clinical Coordinator (RN) you will partner with our Director of Patient Care Services and Interdisciplinary Teams to further that mission.
These passionate teams will depend on you to care for and support our patients during their hospice journey.
The Clinical Coordinator is to be the first line for phone calls coming in from patients and families or field staff.
The Clinical Coordinator must be able to answer questions regarding hospice services, re-filling prescriptions, and assist with ordering supplies and DME.

REQUIRED EDUCATION & EXPERIENCE:
Graduate of an accredited school of nursing
Current RN licensure in a State where providing coverage, CPR certification, and a valid driver’s license
Minimum one (1) year hospice experience
A Bachelor’s degree in nursing may be used in lieu of experience
On an Average day, you will:
Take patient/family phone calls to determine the needs of the caller
Follow up on phone calls by either directing the phone call to the DPCS, RN clinical manager or RN case manager and/or taking care of the need immediately per scope of practice
Re-fill prescriptions, order supplies, order DME, and any other coordination needed by the patient and families
Complete data entry into AllScripts for all patient discharges
