Primary Purpose:
The person in this position is a member of the Patient Centered Health Care Home team where each team member plays an integral role in providing patient-centered health care.
S/he is responsible in assisting the provider and the team in the overall coordination, preparation, examination and treatment of the patient.
S/he will perform the minimum competency requirement for a medical assistant.
Essential Duties:
Reviews patient records prior to the visit
o Reminds patient of appointments as necessary
o Identifies if all previous orders were completed
o Ensures that all necessary reports are in place
o Reviews gaps in care as necessary
Performs check in duties appropriately and timely
o Records vital signs and other measurements accurately
o Updates all information related to meaningful use and PCHCH requirements
o Carries out necessary check in clinical procedures appropriately per protocols
Performs check out duties appropriately and timely
o Carries out clinical procedures and order as directed by the provider efficiently and accurately
o Administers medication as ordered by the provider
o Assists in simple wound dressing as necessary
o Refers patients as necessary
o Complete laboratory orders accurately
Performs administrative duties
o Prepares patient exam room
o Inventories supplies
o Completes necessary forms
o Follows up on no-shows and schedule as necessary
o Sterilizes, labels and packs instruments as necessary
o Retrieves and returns calls from patients, staff and other agencies in the community
o Calls patients for laboratory and/or other reports
o Schedules appointments as necessary
Operates basic medical equipment
Documents consistently and appropriately in EMR
Communicates with the provider and other members of the team to enhance patient care
Regular and reliable on-site attendance is required.
Other Duties:
Performs other related duties as assigned
Attends and participates in PCMH huddles, in-services education programs and meetings as required.
Demonstrates excellent customer services to patients and/or providers and promotes positive image of KPHC.
Provides feedback for quality improvement activities
Arrives to work promptly.
Inform supervisor when late or absent is imminent.
Answers phones within reasonable amount of time and take accurate messages.

Minimum Qualification Requirements:
Skills/Knowledge:
Ability to add, subtracts, multiply, and divides in all units of measure, using whole numbers, common fractions, and decimals.
Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
Ability to deal with problems involving several concrete variables in standardized situations.
Education:
High School Diploma/GED required
Medical Assistant Certification required
One-year certificate from college or technical school; or equivalent combination of education and experience.
Current BCLS/CPR
Experience:
Previous experience with basic computer systems required
Knowledge of basic medical terminology required.
Bi-lingual preferred
