As a Navian Hawaii hospice nurse, you will guide patients and their family on their journey, delivering comfort and support with a caring heart, replacing fear with hope, helping them find the beauty and joy in every moment, and providing each person with the kindness and dignity they deserve.
Primary Responsibilities
Uses clinical judgment and assesses needs of the patient and their families and initiates intervention to meet the immediate needs by utilizing principles of palliative care and following (but not limited to) the following guidelines:
Assesses the need for skilled nursing by phone.
Makes home visits to calm a situation, to provide necessary skilled interventions, and when a patient expires.
When indicated, gives pertinent report to attending physician and/or obtains new medication orders.
Reports any administratively complicated situation to the Patient Care Coordinator
Seeks consultation from patient's primary care physician or Hospice Hawaii Medical Director and implements orders/recommendations.
Makes routine/follow up calls or home visits to new admissions and current patients while on duty, utilizing backup system as needed, in accordance with Hospice Hawai'i's 24 hours per day 7 days per week service and admission policy.
Obtains updated patient information and physician orders by:
Accessing report via the voice mail system or directly from the RN Case Managers
Reviewing patient medical records in the EMR system or
From another On-Call RN with whom the start of work interfaces, whichever is appropriate to the timing of the start of duty hours
Clears calls with answering service within 10 minutes of receiving a phone message.
Documents all phone calls, home visits and medication orders in accordance with Hospice Hawai'i protocol.
Provides clinical report to Interdisciplinary Team regarding all occurrences during after-hours shift and interventions done for those occurrences by 7:00 AM.
If patient calls and/or visits are initiated within the last hour of the shift, call logs must be updated and re-distributed to staff before the end of the work shift.
Assumes the role of either primary or backup call when other After Hours staff is on leave.
Other related duties as assigned by the Patient Care Manager or Director of Clinical Operations.
Shifts:
Every Night:
4:00 PM through 12 AM
12 AM through 8 AM
Weekend Days: 8:00 AM through 4 PM
Experience, Educational & Employment Requirements:
A current Hawaii license, in good standing, as a Registered Nurse.
BSN preferred
At least two years' experience as a staff nurse.
Hospice and/or home health nursing experience preferred.
Must have a current CPR BLS certificate
Valid driver's license and reliable transportation in a registered, insured motor vehicle.
