The Position:
We're looking for a passionate Client Care Manager to join our home health team.
This is a blended role that includes responsibility for marketing, referrals, and intake blended with aspects of Business Development and Care Management.
Other responsibilities include:
Develops and maintains relationships with referral sources, including hospital case managers, assisted living facilities, skilled nursing/rehab facilities, hospices, and physician’s offices for the purpose of increasing referrals and achieving business growth goals.
Responsible for all aspects of the patient intake process to assure safe and appropriate transition to the home environment.
Oversees and directs Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs) when designated.
Leads all business development activities.
Educates referral sources and consumers about agency services.
Demonstrates knowledge of current best practices and evidence-based standards of care related to management of commonly served populations.
Requirements:
Bachelor’s Degree in Nursing from an accredited school of nursing preferred.
Current Registered Nursing license in the State of Hawaii.
BLS Certification.
Three years nursing experience, preferably in Medicare certified home health.
Some home care coordinating or discharge planning experience preferred.
Established relationships with local referral sources highly desirable.
Knowledge of state and federal regulations governing home health services desired.
