Primary Responsibilities:
•Conduct comprehensive bio-psycho-socio-cultural, and bereavement assessments for patients and families receiving
end-of-life care; create and implement therapeutic treatment plan.
•Provides crisis intervention, individual, couple and group therapy to patients and families for a diverse range of issues
related to grief/loss, adjustment to illness, relationships/family dynamics, mental health, abuse, symptom/pain
management, spirituality and bereavement.
•Facilitates case management with referral to appropriate community resources; promotes patient and family advocacy.
•Provides counseling to patients and families regarding end-of-life care issues including medical decision making,
Advanced Health Care Directives, disease process and caregiving; initiates community outreach.
•Participates in the provision of social work services in a transdisciplinary care team setting; contributes/participates
regularly in IDG team conferences and collaborates with team members in developing a patient centered plan of care.
•Participates in regular supervision with Supervisor and attends Trainings.
•Assists in the training and orientation of new hires.
•Performs other duties as required.

Experience and Educational Requirements:
•Masters Degree in Social Work (MSW) from an accredited school of social work
•Possession of a current license to practice as a Licensed Social Worker (LSW) in the State of Hawaii
•At least one year of experience in a related social work field.
Working Conditions:
•Indoors in an air-conditioned office as well as outdoors in all types of weather.
•Travel to patient homes in a variety of settings.
Equipment Used:
•Operates phone system, cellular phone, laptop for documentation (Word, Excel, Microsoft Outlook, Clinical Charting
Software and Email), calculator, copier and postage machines.
•Durable Medical Equipment (DME) such as: hospital bed, wheelchair, walker, commode and shower chair.
•Frequent use of own personal motor vehicle which meets valid safety, registration and insurance requirements.
•Vehicle must be in good operational condition with ability to travel long distances.
Hours:
•Generally Monday through Friday, 8:00 am through 4:30 pm.
However, position may require after hours work including weekends and holidays, as needed.
Physical, Mental and Communication Demands:
•Requires long periods of sitting.
Requires use of hands and must be able to speak, read and write English.
Vision
abilities include close vision, depth perception, and ability to focus.
Must be able to lift and move up to 25 lbs unassisted
and be able to work on multiple tasks simultaneously and meet multiple deadlines.
Able to analyze problems and adopt
an effective course of action.
Requires learning a number of policies and procedures, writing, arithmetic and problem
solving skills.
•In addition to being able to fulfill the physical, mental, and communication demands listed above, must be able to follow safety instructions and demonstrate critical thinking.
